CELC 2025 - DAISY Award

The National Association of Indian Nurses of America (NAINA) is delighted to announce our partnership endeavor with the Disease Attacking the Immune System (DAISY) Foundation to honor outstanding nurses. 
Purpose:
The DAISY award is an award program designed to recognize extraordinary nurses for their compassionate care. In partnership with The DAISY Foundation, NAINA will offer the NAINA-Daisy Health Equity Award to honor an individual nurse and a chapter that demonstrates excellence in bridging healthcare disparities by providing outstanding, compassionate care. This includes working to eliminate health disparities caused by social determinants of health to include but not limited to socio-economic, demographic, cultural, and literacy levels affecting health equity. 
Please review the eligibility criteria and documents required before submitting your application.​
Eligibility Criteria for Nominee: 
• Must be a current and active NAINA member/Chapter
• Nominee contributions include improving health equity and providing compassionate, quality healthcare in any of the following areas

  • Health and Health Care
  • Social and Community Context
  • Education
  • Economic Stability efforts
  • Neighborhood and Built Environment

Eligibility Criteria for Nominator:
May be members of NAINA or patients, family members, nurses, physicians, and other members who have direct knowledge of the contribution to support the nomination based on their experiences or observations of extraordinary, compassionate care.
Should have permission from the nominee to provide his/her information to NAINA and to use the photographs and bio by NAINA.
Application:
Complete the online application before August 01, 2025 
For individual awards - Attach a narrative (not to exceed more than 150 words), a photo, and a short bio of the nominee.
For chapter awards - Attach a narrative (not to exceed more than 150 words), documents, pictures, and audio-visual media that demonstrate the following topics. 

  • Compassionate care
  • Philanthropic contributions and efforts
  • Actions to identify socio-economic and literacy needs of the underserved population.
  • Efforts to integrate social services into holistic care.

Note: The narrative and all the documents should be in one PDF document
Consent:
1. Submission implies consent for the material submitted to be used by NAINA
2. Submission implies permission to NAINA representatives the rights to use photographs of the nominee and Bio for purposes of publicity, illustration, advertising, and Web content in connection with the NAINA awards and scholarships.
3. All entries will become the property of the NAINA and will not be returned.
Exclusion Criteria:
Incomplete and/or late submissions 
Falsification of documents 
Documents must be submitted in separate files or zip files.
Award period: January 2024 to June 2025

Deadline for Submission: August 01, 2025
